Jurat: you swear or affirm that what the document says is true, then sign it in front of the notary.

Tax preparation

What to bring to your tax appointment

Having these ready is the fastest way to a correct return. Not every item applies to everyone, so bring what you have and we will tell you what is missing.

  • Photo ID for you and your spouse
  • Social Security cards or ITIN letters for everyone on the return
  • Dates of birth for every dependent
  • W-2 forms from every employer
  • 1099 forms for contract work, interest, dividends or unemployment
  • Mortgage interest statement (Form 1098) and property tax bills
  • Health coverage forms (Form 1095-A if you used the Marketplace)
  • Last year's tax return, if we did not prepare it
  • Bank routing and account numbers for your refund
  • Childcare provider name, address and tax ID

Send your documents through our secure client portal. Please do not email or text photos of Social Security cards, W-2s or other tax forms, because regular email and texts are not secure.

Prefer to bring paper copies? Add it to your request and we will schedule a visit.

Acknowledgment and jurat certificate

Some documents come without a notary certificate. This is the New Jersey wording we attach, with room for the stamp. The notary completes it at your appointment; signers do not fill it in.

Jurat

Subscribed and sworn (or affirmed) to before me on ____________, 20___, by ____________ (name of affiant).

Each certificate is followed by the notary's signature, printed name, commission expiration date and stamp.
